• Proof of SSS contributions (if relevant).
• Copy of real estate property – title-deed (if relevant).
• PSA Marriage Certificate
• PSA Marriage Certificate (if married)
• Certificate of employment; Certificate of leave absence; Latest Income Tax Return (if employed)
• Business Permits (DTI, Mayors Permit, COR); Latest Income Tax Return (if Self-employed)
• Proof of travel health insurance
